A primary care practice serves as the first point of entry into the healthcare system and as the continual focal point for healthcare services. Equity Advanced Nursing and Wellness Services (EAN) provides health and wellness services that include office-based and home-based visits to residents of personal care homes who, due to their chronic and often debilitating conditions, are unable to travel to an office appointment regularly. Members of the community who are both insured and uninsured, as well as personal care homes within 35 miles of our office.
